Pilot Privacy Notice

What this notice covers

This notice describes information handled through the Digital Life Checkup, pilot enrollment, account, Personal Digital Life Review, and pilot feedback. The pilot is intended for invited adults in the United States.

Information we collect

  • Checkup information: your question responses, scores, section results, and session status.
  • Optional research information: broad age range, household composition, household technology role, and how you heard about the Checkup.
  • Enrollment and contact information: name, email address, mobile phone number, time zone, service selection, and communications consent.
  • Service information: availability, appointment details, questions you want help understanding, adviser interpretations, internal service-delivery notes, Review status and timing, written Review content, and follow-up flags.
  • Feedback: ratings and comments you choose to provide after the Review.
  • Account and technical information: authentication records and routine security or diagnostic logs created when the site and its service providers operate.

Please do not provide passwords, account numbers, security codes, or other account credentials. The pilot does not need them.

Anonymous Checkup and enrollment

You may complete the Checkup without creating an account. Before enrollment, the Checkup is associated with a browser session rather than a customer contact profile. If you choose to enroll using those results, the completed Checkup is associated with your account and Review so your adviser can discuss it with you. The optional research profile remains separated from your customer contact and household profile and is intended for aggregate pilot analysis.

Household Review invitations

If an enrolled participant invites another adult, we use the invited adult's name, email address, invitation status, consent record, and separately completed Checkup to administer one shared household Review. Each adult must use their own verified account and consent separately. The adults may see shared Review summaries and household next steps, but one adult does not receive the other adult's answer-level Checkup responses through the participant experience.

How we use information

  • calculate and display Checkup results;
  • create and secure your account;
  • enroll you, arrange your appointment, and deliver the Review;
  • allow authorized managers and advisers to administer the pilot;
  • send service-related communications;
  • maintain security, diagnose problems, and prevent misuse; and
  • evaluate and improve the pilot, including through aggregate patterns and participant feedback.

We do not sell or rent participant information, and we do not use Checkup or Review information for targeted advertising.

Who may receive information

Information may be available to the pilot administrator, assigned adviser, and other authorized personnel who need it to operate or evaluate the pilot. Hosting, database, authentication, and email-delivery providers process information as needed to run the service. Information may also be disclosed when reasonably necessary to protect the service or participants, investigate misuse, comply with law, or respond to valid legal process.

Retention and deletion

Because the retention schedule is still being finalized for the pilot, we do not yet promise a fixed deletion date. We intend to keep identifiable information only while it is reasonably needed to provide and evaluate the pilot, maintain security, resolve disputes, or meet legal obligations. You may ask to review, correct, withdraw from the pilot, or request deletion of your information. Some information may remain temporarily in protected backups or when retention is legally required.

Security

We use access controls and other reasonable safeguards appropriate to this pilot. No online service can promise absolute security. Protect your sign-in email and password, use a unique password, and tell the pilot administrator promptly if you believe your account or information has been exposed.

Your choices and contact

The optional research questions and written feedback are voluntary. You may choose not to enroll after completing the Checkup. To ask a privacy question or request access, correction, withdrawal, or deletion, reply to a pilot email or use the pilot administrator’s contact details in your invitation. Please do not send passwords or account credentials.

Changes to this notice

We may refine this notice as the pilot develops. The updated date will change when we do. If a change would materially affect identifiable information already collected, we will provide additional notice when appropriate rather than relying only on a quiet website update.
